Patient Aligned Care Team (PACT)
PACT, the VA version of the Patient Centered Medical Home, is a health care delivery model designed to provide care that is Veteran centric, flexible, all-inclusive, results oriented and forward thinking. Each PACT is composed of a primary care provider, registered nurse, licensed practical nurse, and clerical associate. The PACT team is assisted by a psychologist, social worker, dietician, clinical pharmacist and other specialists as needed. Walk-in patients presenting for urgent care are seen by a member of the own PACT, thus providing enhanced continuity of care.
