Health Promotion Disease Prevention Program

Getting the Most Out of My HealtheVet!

The Health Promotion Disease Prevention (HPDP) provides Veterans with comprehensive health education, clinical services and support for health-related behaviors. The HPDP Program works closely with the Patient Aligned Care Team (PACT) and provides resources to support clinical staff and Veterans to address health promotion and disease prevention needs and interests.  The Veterans Health Administration (VHA) National Center for Health Promotion and Disease Prevention (NCP) has developed the Preventive Care Program to ensure a more holistic, Veteran-centered system and greatly improved access to and coordination of care.
