Providing Primary Care Services in the Veteran’s Home

What is Home Based Primary Care?

• Home Based Primary Care (HBPC) is health care services provided to patients in their home.
• A VA doctor directs the health care team who provides the care.
• HBPC is for patients with health problems that make it hard to use and travel to VA Primary Care Clinics.  
• Most health care can take place in the home; such as, check-ups, teaching, getting samples for labs, and managing the patient’s medications. 
• HBPC is a team of experts trained to provide the best possible care for patients at home.

You’ve heard the saying, “There’s no place like home.” Sometimes, home is the best place for your care, and our Home Care team is committed to helping you and your family at home. HBPC provides direct care and education to help you manage your condition and avoid complications. Our mission is to deliver the highest quality patient care experience.

Who provides care in the home?

The team consists of licensed professionals:
• Nurse Practitioners
• Physical and Occupational Therapists
• Dieticians
• Pharmacy representatives
• Social Work Service staff
• Mental Health clinicians

What will home care do for me?

New or chronic medical conditions or illness can be stressful for you and your loved ones. You may need assistance in learning how to manage your illness. We recognize that home is where you feel most at ease, and our loyal team has a passion for healing. We create a plan of care with you and your loved ones to maximize your health and wellness.

How can I get home care?

The home care program is available to Veterans who are eligible to receive care at Central Texas Veterans Health Care System. You must live within approximately 50 miles of the health care system or one of our  outpatient clinics and have a relative or friend who can help with your home care (only if you cannot manage alone).  The HBPC team will come to your home to talk with you and your family to see if you are a good match for the program. The cost for the program depends on your individual VA eligibility. 

How can I get more information?

To start the home care process, you can talk to your regular doctor or give us a call. Once we hear from you, a member from our team will come to your home to give you an overview of our program and together, we will work to match your needs.
